Supporting your independence, dignity and wellbeing

At Care and Carriage, we offer care-led support for everyday life. Whether that’s at home, out in the community, for essential errands, appointments or social activities. Our approach is centred on you as a whole person, not just the journey.

Our trained Community Support Companions are with you throughout the experience, providing practical assistance, reassurance and supervision where needed, so you can participate in life with confidence and dignity.

What We Offer

Assisted Outings & Community Activities

We support a wide range of everyday activities, including:

  • Routine Appointments – Helping you attend scheduled healthcare, wellbeing or community appointments with the support you need.
  • Shopping and Essentials – From groceries to errands, we’re here to assist at every step.
  • Social Events & Family – Weddings, birthdays or get-togethers, support you can rely on.
  • Day Trips & Wellbeing – Whether a day at the coast, park or a special trip, we can help you enjoy it.

All activities we support are connected to your independence, wellbeing and confidence, not just transport alone.

We understand that people need different kinds of support.

So rather than a one-size-fits-all approach, we offer three levels of support designed to match individual needs.

Support-Based (SB)
Everyday support to boost independence

For people who are able but benefit from reassurance, presence, or practical support.

This may include:

  • Support at home
  • Accompaniment on foot or using public transport
  • Support alongside individuals own transport arrangements
  • Attending appointments or activities with a supportive companion

This tier does not include use of a Care & Carriage vehicle.

Transport-Supported (TS)
Supported access to the community

For people with physical, sensory, or mental health needs who benefit from additional support to access the community.

This may include:

  • Use of a Care & Carriage vehicle
  • A single Community Support Companion
  • Planned support linked to appointments, wellbeing, or daily life

Transport is provided as part of support, not as a standalone service.

Enhanced / Accompanied (EA)
Higher-complexity support

For people who require additional supervision or shared responsibility.

This may include:

  • Individuals with advanced dementia
  • People with high-support autism
  • Children and young people under 18

This tier requires the individual to be accompanied by a parent, guardian, or existing carer.

Support may include use of a Care & Carriage vehicle where appropriate.

Is This Service Right for You?

This support is most suitable for people who:

  • Have a care or supervision need that makes independent journeys or activities challenging
  • Require consistent practical support to feel safe and confident
  • Would find standard transport or independent travel unsafe or unsuitable

If you’re unsure, our eligibility check and assessments are there to guide you.

Meet Your Community Support Companion

Our Community Support Companions are:

  • First Aid trained
  • Enhanced DBS checked
  • Trained in safeguarding and dementia awareness
  • Skilled in assisting with mobility and safe transfers

They are there to support you as a person, not just during travel.
