Care & Community Support
Supporting your independence, dignity and wellbeing
At Care and Carriage, we offer care-led support for everyday life. Whether that’s at home, out in the community, for essential errands, appointments or social activities. Our approach is centred on you as a whole person, not just the journey.
Our trained Community Support Companions are with you throughout the experience, providing practical assistance, reassurance and supervision where needed, so you can participate in life with confidence and dignity.

What We Offer
Assisted Outings & Community Activities
We support a wide range of everyday activities, including:
- Routine Appointments – Helping you attend scheduled healthcare, wellbeing or community appointments with the support you need.
- Shopping and Essentials – From groceries to errands, we’re here to assist at every step.
- Social Events & Family – Weddings, birthdays or get-togethers, support you can rely on.
- Day Trips & Wellbeing – Whether a day at the coast, park or a special trip, we can help you enjoy it.
All activities we support are connected to your independence, wellbeing and confidence, not just transport alone.
Care Supported Transport
Transport is part of how we help — but it’s never just about moving from A to B. If you require additional assistance or supervision during travel, we provide:
- Comfortable, accessible transport with support throughout the journey
- Assistance with boarding, mobility and securing any equipment
- Help managing handovers, wayfinding and understanding appointment logistics
- Time-keeping support to reduce anxiety about delays or missed connections
This isn’t a standard taxi or private hire situation — you’re supported by someone trained to help you feel safe and in control.
Support Around Appointments and Recovery
For many people, going to and from healthcare settings — like hospitals, clinics or therapy sessions — requires more than just a vehicle. We provide:
- Appointment assistance — support before, during and after your visit
- Discharge support — helping people transition home safely after a period in care
- Non-urgent travel support — where additional reassurance or physical help is needed
Our focus is on your comfort, confidence and continuity of care throughout.
Support at Home and in the Community
You don’t have to be leaving the house for us to support you. Our Community Support Companions can help with:
- Household tasks (light support to help you stay independent)
- Companionship and encouragement (shared activities, outings or encouragement to stay engaged with life)
- Motivation and confidence building (avoiding isolation or anxiety about going out)
We’re here to help you live well — not just move around.
